I wish to participate in dragon boating and associated activities.
Membership.
By registering for or participating in the dragon boat event:
· you will become a member of the Dragon Boat Queensland and by extension the Australian Boat Federation for the period of the event.
· You agree to be bound by the policies of Dragon Boat Queensland including the National Integrity Framework and Fair Play Code that cover such matters as Child Safety, Member Protection, Competition Manipulation anti-doping and the standards of behaviour expected by everyone involved in sport in Queesland

Fitness and Medical Treatment Consent
I certify that I am medically and physically fit and able to participate in dragon boating and associated activities. I say that I have received sufficient training for participation in the event and have not been advised otherwise by a qualified medical person.
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and or illness during this event.
I acknowledge that this declaration will be used and relied upon by The Club, and Dragon Boat Queensland Inc., event holders, sponsors and organizers of events in which I may participate as evidence of my fitness and ability to participate and I agree to be bound by it.
Risks of this Sport
I acknowledge that:
1. Dragon boating can be an inherently dangerous sport; and
2. The risks of serious injury, death or accident equipment are not only inherent to paddlers, but are also present for volunteers and may arise from:
a. Dangerous or defective equipment;
b. Property owned, maintained or controlled by the Club, the Dragon Boat Queensland Inc. or event organizers / holders; and
c. A person’s or group or people’s negligence or carelessness.
I have voluntarily read this warning and understood this warning and accept and assume all the risks inherent or other wise of participating in dragon boating, associated activities of The Club and/or volunteering in this event.
I acknowledge that this Waiver and Release of Liability form will be used and relied upon by The Club, Dragon Boat Queensland Inc., event holders, sponsors and organizers of events in which I may participate and I agree to be bound by it.
In consideration of permitting me to participate in this event, I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
(A) Waive, Release and Discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may here after accrue to me or my travelling to and from this event,except where such loss, damage or injury is suffered because of the misconduct or negligence of THE FOLLOWING ENTITIES OR PERSONS: Dragon Boat Queensland Inc. (DBQ), The Club, DBQ &/or Club’s directors, officers, employees, volunteers, representatives and agents, the event holders, event sponsors, event directors and event volunteers; and
(B) Indemnify and Hold Harmless the entities or persons mentioned in paragraph (A) from all liabilities or claims made by other individuals or entities as a result of any of my actions during this eventexcept where such liabilities or claims that arise because of misconduct or negligence of the entities or persons mentioned in paragraph (A)
This Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
